5 Our rabbi the Beit Yosef wrote in paragraph 3 that the custom is that the prayer leader may, when he wishes, recite the blessing and read without taking permission, because it is as if from the time they appointed him as prayer leader they gave him permission for this. These are his words. And our rabbi the Rema wrote regarding this that in these countries they do not practice this way, and the cantor does not ascend except when the gabbai tells him to ascend. However, they do not call him by his name like the other people called to the Torah, whom they call by their names "so-and-so son of so-and-so." These are his words. For in the country of our rabbi the Beit Yosef, the prayer leader calls all those who are called up on his own initiative, and therefore it is as if they gave him permission to ascend on his own, for if not, who would call him? But in these countries, the prayer leader only calls those whom the gabbai tells him to call, and therefore the gabbai also calls him [differently] — it is not appropriate to call him [in the usual way], since he is standing by the Torah scroll and it is not appropriate to say "let him stand" since he is already standing here. Rather, [the gabbai] tells him that he will be among those called up. (This seems to me to be the reason based on Darkei Moshe section 1 and the Vilna Gaon subsection 5.)
